What Root Canal Retreatment Actually Is

Root canal retreatment is the process of removing the filling material from a tooth that has already had a root canal treatment, disinfecting the canal system again, and refilling it. It is not the same as a first time procedure.

 

The canals have already been shaped and filled, so the challenge is different: the dentist has to safely remove old material, find whatever was missed the first time, and treat it without weakening the tooth further.

Signs You May Need Retreatment

A failing root canal does not always hurt straight away. Some patients notice nothing until a routine X ray shows a problem. Watch for:

  • Returning pain or tenderness when biting on the tooth

  • Swelling or a small pimple like bump on the gum near the tooth

  • A tooth that feels different from the surrounding teeth

  • New sensitivity or a dull ache that comes and goes

The Two Approaches to Retreatment

Not every failed root canal is treated the same way. The right approach depends on where the problem sits.

Non Surgical Retreatment

This is the standard and most common approach, sometimes called orthograde retreatment. The dentist works through the top of the tooth, exactly as in the original procedure. It is chosen when the infection is contained inside the canal system and the issue is something like a missed canal, a broken instrument, a perforation, or a partially completed treatment.

Surgical Retreatment

When the source of the problem sits outside the canal system, or when working through the top of the tooth is not feasible, surgery becomes the better route. This is known as apical surgery or apicectomy. It is used for cysts, foreign material beyond the root tip, extra canal infection, or a blockage that cannot be bypassed.
