What Is Apical Therapy?

Apical surgery is a surgical approach to endodontic treatment where the dentist accesses the root tip directly through the jawbone rather than from the tooth crown.

 

The "apex" refers to the tip of the tooth root. During the procedure, a small surgical window is created in the cortical bone overlying the root, infected and inflamed tissue around the root tip is removed, and the apex itself may be trimmed.

​This differs fundamentally from standard root canal treatment, which approaches the tooth from the crown downward. When that conventional pathway fails or is blocked, apical surgery offers an alternative route to eliminate infection and restore tooth health.

Contraindications and When to Extract Instead

Not every tooth is a candidate for apical surgery. According to endodontic guidelines, surgery should not be pursued when:

  • Local anatomical factors make the root end inaccessible (very deep bone, proximity to vital anatomy)

  • The tooth cannot be made functional or restorable (poor crown to root ratio, inadequate ferule)

  • Vertical root fractures are present

  • Severe periodontal disease exists with clinical attachment loss to the apex

  • The tooth has insufficient bone support to support a crown restoration

 

In these cases, extraction and replacement with a dental implant or bridge may offer a better long term prognosis than multiple surgical attempts.
